For those of you who are new readers of these blog posts, the Ibis Program is a self-management support program for individuals with multiple chronic conditions who need support for self-care. When individuals join the program, they become Members of the Ibis community. Members receive an interactive device that guides their self-care. The population that we are most focused on is socio-economically disadvantaged seniors: the dually eligible population receiving benefits from both Medicare and Medicaid.
