By gathering and analyzing data real time, Ibis enables clinicians to optimize healthcare services delivery and empowers residents to self-manage their chronic diseases, which leads to a triple-aim effect of better patient experience, improved population health and reduced cost via improved care plan adherence.
ElderCare Network of Lincoln County consists of seven home centers that function like regular households and provide attentive and personalized care and emotional support to elders, while fostering social engagement, intellectual stimulation and as much independence as possible to Lincoln County elders, whatever their income.
