For those of you who are new readers of these blog posts, the Ibis Program is a self-management support program for individuals with multiple chronic conditions who need support for self-care. When individuals join the program, they become Members of the Ibis community. Members receive an interactive device that guides their self-care. Members are also assigned a Member Advocate who supports the Member with staying on track with self-management. The Ibis Program is powered by a unique artificial intelligence (AI) that provides daily self-care guidance to our Members. When the AI hits a roadblock, a person who we call a Member Advocate takes up the responsibility to address the health or social challenges faced by our Members. The population that we are most focused on is socio-economically disadvantaged seniors: the dually eligible population receiving benefits from both Medicare and Medicaid. Comments are closed.
