IbisHubThe two way communication platform for Members is the AI-based IbisHub that is proven to enhance communication and engagement between Members and their Care Team. Self-care support is personalized and directed by the Member’s clinician approved protocols and delivered through a dedicated terminal in the home. Members enter data though the IbisHub yielding timely information on medication adherence, meals, exercise, mood and vitals.

IbisNexus is a cloud connected Web application that provides a range of tools to remotely monitor and manage the health of Ibis Members. Changes to the care plan for medication, diet, exercise, and self-monitoring regimens are made by the Ibis team through IbisNexus based on information and instructions from the Ibis clinicians. The care plan’s details, including any revisions, are delivered to the patient every day via the IbisHub.
Through the IbisNexus, all care providers can review data collected by the Ibis to evaluate the health trends and review day-to-day developments of members under their care. With IbisNexus, the relevant care team personnel have deep, easily accessible, and insightful data about their patients at their fingertips. |
