Whole Team Engagement

Seamless, Constant Communication Drives Engagement, Adherence and Timely interventions

The Ibis communication and engagement technology is directed at enhancing communications and engagement with both the end user patient and the clinical care team, be it from Senscio and/or from the provider partner/client of Senscio.  The communication platform for patients is the IbisHub™; the communication platform for the clinical care team and client is the IbisNexus™.  Ibis creates a "virtual community" among the persons served and we use the term Member to define the individual being supported by the Ibis solution.​

IbisHub

The two way communication platform for Members is the AI-based IbisHub that is proven to enhance communication and engagement between Members and their Care Team.  Self-care support is personalized and directed by the Member’s clinician approved protocols and delivered through a dedicated terminal in the home.  Members enter data though the IbisHub yielding timely information on medication adherence, meals, exercise, mood and vitals.

IbisNexus

​IbisNexus is a cloud connected Web application that provides a range of tools to remotely monitor and manage the health of Ibis Members.  Changes to the care plan for medication, diet, exercise, and self-monitoring regimens are made by the Ibis team through IbisNexus based on information and instructions from the Ibis clinicians.  The care plan’s details, including any revisions, are delivered to the patient every day via the IbisHub. 
Through the IbisNexus, all care providers can review data collected by the Ibis to evaluate the health trends and review day-to-day developments of members under their care.  With IbisNexus, the relevant care team personnel have deep, easily accessible, and insightful data about their patients at their fingertips.

Meaningful and Timely Analytics

 Data and analysis  is reported on both an individual and population based level.  Easily interpreted reports enable the care team to assess self-management effectiveness, adherence, behaviors, risk stratification, and outcome trends as examples.​ 
Individual Level
Clinicians can view all data collected by IbisNexus to see their patients’ health trends and can drill down to see detailed day-by-day information. Clinicians can receive reports that they can quickly review to become aware of:
  • Recent changes in health or self-care habits
  • Medication changes
  • Acute care utilization

Population Level
Clinicians can see a panel level dashboard on the health and care management status of all their patients to quickly guide actions. The dashboard also allows for tasking and tracking of care management activities.
