The late Dr. Hugh F. Stoddart, co-founder of Senscio Systems Inc., experienced the challenges of navigating the healthcare system as a patient with congestive heart failure first hand. Following a heart valve replacement and subsequent infection, Hugh was stripped of his independence and lived the rest of his days battling the obstacles our healthcare system placed in front of him. Hugh and his fellow co-founders and family members, Dr. Piali De and Dr. Hugh A. Stoddart, quickly realized that the healthcare industry becomes exponentially more complicated if you are not in pristine health. This realization is central to Senscio’s mission: to challenge and innovate the current way we deliver care for patients with complex health.
The Ibis Program demands changes in the United States’ healthcare. By ignoring the frailties in our chronic care delivery model, we disable patients with chronic conditions from reaching their highest potential, a fundamental right that Senscio Systems’ believes every individual deserves. Through the insights generated by Ibis, we can create an environment that enables both patients and healthcare providers to create a truly individualized treatment plan. The Ibis Program is responding to a broken chronic care system:
Clinicians do not have the resources needed to provide the support required for patients with multiple chronic conditions.

1. The IbisHub: Our easy to use in-home touch screen monitor provide a series of reminders on a daily basis to promote the best habits for self-managing chronic conditions. Our members report their health and medication adherence daily. When a member reports their health may be shifting, Ibis Care Navigators are prompted.
2. Ibis Member Advocates: Our Member Advocates serve a variety of functions anchored by the needs our members may have in their Ibis self-management journey. Member Advocates check in routinely, connect members to community resources, ensure their IbisHub is functioning properly, and are first responders to non-emergent health and/or behavioral shifts. 3. Ibis Chronic Care Clinicians: Our Ibis clinicians generate a patient centered care plan that is delivered through the member’s IbisHub. They respond to health shifts detected by Ibis and coordinate accordingly with other clinical care providers.
